    UroLift® Center of Excellence (COE) 

    COE

    The UroLift® Center of Excellence (COE) Program is designed to enhance patient outcomes and experience. Urologists included in the UroLift COE Program have demonstrated a commitment to care for men suffering from symptoms associated with Benign Prostatic Hyperplasia (BPH) or an enlarged prostate and achieved a higher level of training with the UroLift System.

    Eligibility and Requirements

    Below are the requirements to meet the COE Designation criteria.
    Interested physician must complete: 

    • UroLift Proficiency Pathway
      The UroLift Proficiency Pathway training — the surgeon training program that includes online module training and completion of 15 UroLift System cases.

    • Physician Education*
      A minimum of two Teleflex Clinical and Medical Affairs (CMA) led educational events. See more information below

    • Case Experience*+  
      A minimum of 60* UroLift System cases within the 12 months prior to submission for designation.

    • IPSS Data Collection*
      Submit de-identified IPSS data from a minimum of 30 UroLift System treated patients including:
      • Pre- and post-procedure IPSS data demonstrating mean improvement ≥40% compared to baseline average
      • Date of each pre- and post-procedure assessment
      • Indicate the use of BPH medication at the time of pre- and post-procedure IPSS assessments

    If you currently are a UroLift COE, and are looking to get re-designated prior to the deadline, the requirements are as follows:

    • Physician Education*
      Physician must complete a minimum of one Teleflex Clinical and Medical Affairs (CMA)-led educational event.

    • Case Experience*+ Physician must complete a minimum of 60 UroLift System cases.

    • IPSS Data Collection*
      Physician must submit de-identified IPSS data from a minimum of 30 UroLift System treated patients including:
      • Pre- and post-procedure IPSS data demonstrating mean improvement ≥40% compared
      • to baseline average
      • Date of each pre- and post-procedure assessment
      • Indicate the use of BPH medication at the time of pre- and post-procedure IPSS assessments

    * Within 12 months prior to submission for designation
    + Cases cannot include a physician’s initial 15 training cases

    If a physician was first designated between June 30th and December 31st, 2020, he/she is required to complete the redesignation requirements for 2022 by December 30th 2021.

    If a physician does not retain their designation as a UroLift COE physician, the physician’s COE designation will be removed. The physician will be reviewed to determine whether they still meet the requirements to remain on the Physician Locator. If the physician does not meet the Physician Locator requirements, the physician will also be removed from the Physician Locator entirely.

    Submission Guidelines

    To submit for COE designation or re-designation, please follow these three steps: 

    1. Complete and submit the COE Program Submission Form 

    2. Using the COE Program Tracker, track the module(s) you have completed. Follow the instructions for submitting this MS Excel (.xls) file

    As a reminder, please remove any protected health information (PHI), including medical record numbers (MRNs) before sending to NeoTract | Teleflex. In the event that any submission contains PHI, the submission will be rejected and returned to the sender. 

    Please Note:

    1. Submitting the data is mandatory. If you do not or are unable to submit the data by the specified deadline, your COE status will be suspended until your data has been submitted, reviewed and approved. 

    2. You are required to fill out both the new COE Submission form and IPSS Tracker. Please use the approved tracking tools IPSS Tracker. See Resources and Tools

    Qualifying Physician Education Programs

    Physician education events will be led by the Clinical and Medical Affairs (CMA) division within Teleflex Incorporated. CMA is focused on providing medical education to support the safe and effective use of Teleflex products in patients.

    The beginning of the period for completion of physician education will coincide with the beginning of the period for case completion. Physician education events do not need to be completed during the same months used for case assessment. In other words, if a physician attended an education event in June 2020 but did not select that month as one of the 12 months for case assessment, the education event in June would still qualify.  

    Approved physician education events (may occur virtually or in-person) and modules (virtually) are subject to change as new information is released.  

    x Physicians must be logged into their individual UroLiftUniversity account for the modules to be counted as complete.
