The list of physicians generated by your search criteria is based on the ZIP code or city that you entered. You will have the option to sort search results by proximity to the ZIP code or city that you entered or by listing UroLift® Center of Excellence (COE) physicians at the top. UroLift® COE physicians have fulfilled the designation requirements for the UroLift® COE Program.
The information and content in the Physician Locator are provided solely by the physicians. This list was created to support patients who want to learn more about the UroLift® System by helping connect them to physicians who currently have demonstrated qualifications and interest in providing quality care with the UroLift® System. All physicians have been given the choice to enable a link to their email address within the listing associated with their name.
Physicians listed on the Physician Locator, with or without COE Program designation, pay no fee for inclusion. Some physicians on this list may purchase products from, provide consulting services to and/or be a party to a co-marketing agreement with Teleflex. Teleflex makes no representations or warranties regarding, and shall not be responsible for, the competencies or skill level of any of the physicians listed on the Physician Locator or designated as COEs or the quality of their procedural outcomes. You and your physician must determine if the UroLift® System treatment is the right procedure for you.
NeoTract | Teleflex reserves the right to remove and/or exclude any physician from the Physician Locator, with or without COE Program designation, immediately and without notice in its sole discretion in the event that NeoTract |Teleflex becomes aware of (i) any alleged or suspected fraud or abuse or violation of law, arising out of any activity involving the physician, (ii) any alleged or suspected violations of any federal or state health care program requirements by the physician, (iii) any threatened, proposed, or actual revocation of license/certification, suspension, exclusion, or debarment of the physician, (iv) any subpoena, legal process, civil investigative demand, discovery request, or similar request for information involving the physician, and/or (v) any facts, allegations or situations of a similar nature.
Physician Locator Information and Requirements
The current requirements to be listed on the Physician Locator consist of:
- Performing, on average, at least 3 cases per month in the last 5-month period, and
- Completion of the UroLift® Proficiency Pathway, which is a surgeon training program launched in March 2017 that includes module, didactic and hands-on device training and completing 15 cases.
Physicians currently listed on the Physician Locator who were trained prior to creation of the UroLift® Proficiency Pathway received equivalent surgeon training to the requirements outlined above, including instructional didactic sessions, clinical video review and hands-on UroLift® System use. The current and prior training programs focus on identifying appropriate patient candidates; preparation and operation of the UroLift® System; and surgical technique.
To remain on the Physician Locator, a physician must:
- Complete at least 5 cases within the last 6-month review period, and
- Complete at least one physician education module during the calendar year.
The case completion criterion is assessed every six calendar months, with the review periods occurring January 1– June 30* and July 1 – December 31. The education module completion criterion is assessed at the end of the calendar year. A physician will be removed from the Physician Locator if they have not met the above requirements. A physician will be added back to the Physician Locator once they have completed at least 5 cases within the last review period and have completed an education module during the calendar year.
*Due to ongoing COVID-19 restrictions, the review period from Jan 1 - June 30, 2021 will not take place.
UroLift Center of Excellence (COE) Program Information and Requirements
The UroLift® Center of Excellence (COE) Program is designed to enhance patient outcomes and experience. Urologists included in the UroLift® COE Program have achieved a higher level of training with the UroLift® System and demonstrated a commitment to care for men suffering from symptoms associated with Benign Prostatic Hyperplasia (BPH) or an enlarged prostate.
Our UroLift® COE designation criteria changed effective January 15, 2021. All those achieving UroLift® COE designation on or after January 15, 2021 are required to meet the following criteria.
- Completion of the UroLift® Proficiency Pathway, which is a surgeon training program launched in March 2017 that includes module, didactic and hands-on device training and completing 15 cases;
- Participation in a minimum of two Teleflex Clinical and Medical Affairs-led educational events for initial designation and one Teleflex Clinical and Medical Affairs led educational events per year for redesignation;
- Completion of a minimum of 60 UroLift® System cases in the 12-month period* prior to submission for designation**; and
- Submission of de-identified pre- and post-procedure patient International Prostate Symptom Score (IPSS) data from a minimum of 30 UroLift® System cases completed in the 12-month period* prior to submission for designation**, evidencing:
- A mean improvement in IPSS of at least 40% compared to baseline average;
- The dates of the pre- and post-procedure assessments; and
- Any BPH medication used at the time of the pre- and post-procedure assessments.
Those achieving UroLift® COE designation prior to January 15, 2021 were required to meet the following criteria:
- Completion of the UroLift® Proficiency Pathway, which is a surgeon training program launched in March 2017 that includes module, didactic and hands-on device training and completing 15 cases;
- Participation in a minimum of two Teleflex Clinical and Medical Affairs-led educational events per year;
- Completion of a minimum of 60 UroLift® System cases in the 12-month period* prior to submission for designation**; and
- Submission of de-identified pre- and post-procedure patient International Prostate Symptom Score (IPSS) data from a minimum of 30 UroLift® System cases completed in the 12-month period* prior to submission for designation**, evidencing:
- A mean improvement in IPSS of at least 40% compared to baseline average.
If a physician does not retain their designation as a UroLift® COE physician, the physician’s COE designation will be removed and that physician will be reviewed to determine whether they meet the requirements to remain on the Physician Locator. If the physician does not meet the Physician Locator requirements, the physician will be removed from the Physician Locator entirely.
*Please note, in situations where elective procedures were not allowed due to COVID-19, those months will not be included in the 12-month period for designation or re-designation. Therefore, the assessment period may not be consecutive months.
